During the viral illness, Edith reported a fever, productive cough, increased sputum production and shortness of breath. Edith has been unable to get any rest at home due to these symptoms.
Background: Edith is a 68-year-old female who has a past medical history of Chronic Obstructive Pulmonary Disease (COPD), hypertension (HTN) and atrial fibrillation (AF). Edith was diagnosed with COPD 8 years ago and has a FEV1 of 55% predicted from 1 month ago. Edith’s regular medications are: Symbicort (100mcg/6mcg) 2 puffs BD, Ipratropium (2 puffs) QID, Metoprolol 50mg daily and Rivaroxaban 20mg daily. Edith comes from home alone following the death of her husband 3 years ago but has the support of her two daughters close by. Edith used to be a keen bingo player and a part of the local craft group. However, due to worsening COPD symptoms, she is not able to participate as much as she would like and now relies heavily on community services to help with her activities of daily living (ADLs).
Assessment: At the commencement of your shift, you undertake a comprehensive focused patient assessment on Edith and find the following:
Body System Assessment Assessment Data
Neurological (CNS) GCS 15 (E=4, V=5, M=6)
Temp 38.0°C
Pain 4/10 (Numerical)
Cardiovascular (CVS) BP 138/82mmHg
HR 92bpm (irregular)
Respiratory SpO2 92% on 2L via NP RR 28bpm
Genitourinary/Renal IDC insitu
Urine Output 20mL/hour
Endocrine/Metabolic BGL 5.3mmol/L
Integument PIVC to L) cubital fossa
Musculoskeletal
Psychosocial/other Lives at home alone
Helpance needed with ADL’s
2 daughters close by
Unable to do social activities that Edith enjoys
Recommendation: Provide safe priority nursing care for Edith.
Patient Problems:
Actual Patient Problems Potential Patient Problems
Impaired gas exchange Risk for ineffective coping
Ineffective breathing pattern Risk for imbalanced nutrition
Impaired urinary elimination Risk for knowledge deficit
Decreased activity tolerance Risk of infection
Ineffective airway clearance Risk of social isolation
Task Description:
Based on the case scenario and body system assessment data above, apply the nursing process to develop a comprehensive nursing care plan for this patient.
1.) She has a medical history of Chronic Obstructive Pulmonary Disease (COPD), hypertension, and atrial fibrillation. The purpose of this assessment task is to develop a comprehensive nursing care plan for Mrs Edith Johnson, taking into account her health history, current presentation, and associated patient problems.

Problem Identification:

Actual Patient Problems:

Impaired gas exchange:
a. Fever, productive cough, increased sputum production, and shortness of breath.
b. SpO2 of 92% on 2L via NP, RR 28 bpm.
Ineffective airway clearance:
a. Productive cough, increased sputum production, and shortness of breath.
b. SpO2 of 92% on 2L via NP, RR 28 bpm.
Potential Patient Problems:

Risk for ineffective coping:
a. Lives at home alone.
b. Unable to do social activities that Edith enjoys.
Risk for imbalanced nutrition:
a. Decreased appetite.
b. Poor dietary intake.
With reference to evidence-based literature, the relevant aetiology, pathophysiology, and defining characteristics associated with each of the four patient problems are explained below:

Impaired gas exchange:
Impaired gas exchange is the inadequate delivery or uptake of oxygen and carbon dioxide (Doenges, Moorhouse & Murr, 2019). Mrs Edith Johnson’s viral illness has resulted in the production of sputum, increased cough, and shortness of breath, leading to the impaired exchange of gases. Impaired gas exchange in COPD patients is due to the decreased elasticity of the lungs, narrowing of airways, and destruction of lung tissue (Global Initiative for Chronic Obstructive Lung Disease, 2021).

Ineffective airway clearance:
Ineffective airway clearance occurs when secretions are not cleared from the airways, leading to increased cough, sputum production, and shortness of breath (Doenges et al., 2019). The viral illness and COPD in Mrs Edith Johnson have led to the accumulation of mucus, making it difficult to clear her airways.

HNN329 Chronic Illness & Supportive Care Assessment Task 1: Care Plan Equivalent 1500 words – 15% weighting Purpose of assessment task This assessment task will…

[10099,10681,7636,615,10892,10680,10891
10553,10616,10254,10896,10897,10247,10295,10898,10893,10894,10895,10899,10551
post-127511","post","type-post","status-publish","format-standard","hentry","category-buy-essay","category-cheap-dissertation-help-online","category-cheap-essay-writer-usa","category-homework-help-writing-service-usa","category-page-paper","category-university-dissertation-writing-service","category-write-my-assignment-ireland","tag-1-assignment-help-online-service-for-students-in-the-usa","tag-australian-best-tutors","tag-can-someone-write-my-assignment-for-me","tag-case-study-in-nursing-writing-a-nursing-case-study-essay","tag-free-nursing-case-studies","tag-help-me-write-my-dissertation","tag-homework-for-you","tag-nursing-case-study-assignment-examples","tag-nursing-case-study-examples","tag-nursing-case-study-presentation-example","tag-nursing-case-study-questions-and-answers","tag-student-nurse-case-study-assignments","tag-write-my-assignment-help-for-college-students
["GET
