Below are your first\u00a0case you will be discussing. Make sure you provide a full diagnosis including any specifiers that may be relevant. I will be monitoring your work carefully for this first group of cases.<\/span><\/p>\n

The Case of Aleta Austin\u00a0
<\/span><\/span><\/strong>Aleta Austin is a 26-year-old woman who was seen by\u00a0<\/span>an LPC during her first clinic visit. When asked what brought her in, Aleta explained, “I work as an aide in a nursing home, and I was lifting a patient up from a chair to her wheelchair. Well, anyhow, all of a sudden I felt this terrible and very painful snap in the back of my neck. I lost my balance\u00a0and fell on the floor. As I was falling, I hit my arm real\u00a0<\/span>bad, too. The next thing I knew, I was in the hospital, and they were doing some X-rays and an MRI. The doctors said I sprained my neck, and I might have a\u00a0hairline<\/span>\u00a0fracture of my arm. They put some kind of neck brace on me and said I had to wear it for two weeks. I\u00a0didn’t<\/span>\u00a0need to have a cast on my arm, but I was supposed to be real careful.”\u00a0<\/span><\/span><\/span><\/p>\n

Aleta continued, “I wasn’t allowed to work for six weeks, so I got workmen’s comp\u2014but the checks stopped when the doctors\u00a0said<\/span>\u00a0I\u00a0could<\/span>\u00a0go back to\u00a0work. It has been four months, and they say\u00a0<\/span>that’s<\/span>\u00a0too\u00a0<\/span>long<\/span>\u00a0to have\u00a0<\/span>pain<\/span>\u00a0for the\u00a0<\/span>injury<\/span>\u00a0I got. So now I have\u00a0<\/span>money problems on top of everything else but I don’t care what they say, I still hurt.” Apparently without\u00a0realizing<\/span>\u00a0it, Aleta was rubbing her neck\u00a0<\/span>while<\/span>\u00a0she was\u00a0<\/span>talking.<\/span>\u00a0She went on, “After being discharged by those\u00a0<\/span>doctors, I went to see several new ones. I thought, you\u00a0<\/span>know, maybe the\u00a0first<\/span>\u00a0ones\u00a0didn’t<\/span>\u00a0know what they\u00a0were\u00a0<\/span>talking<\/span>\u00a0about.”<\/span><\/span><\/span><\/span><\/p>\n

The LPC asked what other doctors Aleta\u00a0<\/span>had<\/span>\u00a0seen.<\/span><\/span><\/span><\/p>\n

“Well, over the past three months,” she replied, “I saw two orthopedic surgeons, one neurologist, two chiropractors,\u00a0and<\/span>\u00a0a massage therapist. I was\u00a0given\u00a0<\/span>lots of different medicines\u00a0and<\/span>\u00a0treatments, but I\u00a0still\u00a0<\/span>didn’t<\/span>\u00a0get much relief. One of the doctors\u00a0<\/span>said<\/span>\u00a0I\u00a0should\u00a0<\/span>see a\u00a0<\/span>shrink.<\/span>\u00a0He thought my problems were\u00a0<\/span>all<\/span>\u00a0in my\u00a0<\/span>head. Can you beat\u00a0<\/span>that?”<\/span>\u00a0Aleta sounded very\u00a0<\/span>annoyed. “That\u00a0<\/span>really<\/span>\u00a0steamed me\u2014doesn’t he realize\u00a0<\/span>I\u00a0<\/span>really<\/span>\u00a0want to go back to work\u00a0<\/span>and<\/span>\u00a0see\u00a0<\/span>all<\/span>\u00a0my friends\u00a0<\/span>again? Not only that, does he think I\u00a0like\u00a0<\/em>being in constant\u00a0pain?<\/span>\u00a0Does he\u00a0think<\/span>\u00a0I\u00a0like\u00a0<\/em>taking<\/span>\u00a0all<\/span>\u00a0this\u00a0<\/span>medicine and not\u00a0<\/span>getting<\/span>\u00a0any\u00a0<\/span>relief<\/span>\u00a0from\u00a0<\/span>any<\/span>\u00a0of\u00a0<\/span>it?”<\/span><\/span><\/span><\/span><\/p>\n


The\u00a0<\/span>practitioner<\/span>\u00a0asked\u00a0<\/span>Aleta<\/span>\u00a0about the medica\u00ad<\/span>tions<\/span>\u00a0she is currently using.<\/span><\/span><\/span><\/span><\/p>\n


She answered,\u00a0“In<\/span>\u00a0the beginning, they gave me\u00a0some\u00a0<\/span>pain<\/span>\u00a0pills<\/span>\u00a0that helped some,\u00a0<\/span>but<\/span>\u00a0the doctors\u00a0<\/span>won’t prescribe\u00a0<\/span>anything<\/span>\u00a0for me now. They\u00a0<\/span>think<\/span>\u00a0I’m\u00a0<\/span>getting too used to\u00a0all<\/span>\u00a0the\u00a0pills.<\/span>\u00a0Now I can only\u00a0take\u00a0<\/span>over-the-counter stuff\u00a0<\/span>like,<\/span>\u00a0you know,\u00a0<\/span>aspirin,<\/span>\u00a0Aleve,\u00a0<\/span>Tylenol, Motrin\u2014but they don’t do much good. Nothing seems to\u00a0help<\/span>\u00a0nowadays.”<\/span><\/span><\/span><\/p>\n

When asked how she spent her days, she replied,\u00a0“I pretty much stay inside the house all day long. I have\u00a0<\/span>a hard time finding a comfortable place to sit or lay down. My brother, Chad, brought over his favorite recliner chair for me to use, and that does help some. Most of the time I watch TV, sleep, or play with my dogs.” Aleta volunteered that she and her husband are “in the middle of getting a divorce after being married\u00a0five years.” She stated, “It was his idea, but I guess it’s\u00a0<\/span>all for the best. During the time we were married, I\u00a0realized that I am a lesbian, so\u00a0<\/span>…<\/span>\u00a0I always felt different\u00a0<\/span>from other women and\u00a0a\u00a0<\/em>little confused about this. I\u00a0tried really hard to\u00a0<\/span>just<\/span>bury these feelings\u2014my family\u00a0<\/span>sort of freaked out. It’s so hard to come to terms with\u00a0this. . . . But I can’t believe he wants a divorce after all\u00a0<\/span>the time we’ve been together. He doesn’t know about me being gay,\u00a0and<\/span>\u00a0he told me he isn’t seeing anyone\u00a0else\u2014he\u00a0<\/span>just<\/span>\u00a0said he was tired of me being sick\u00a0<\/span>all<\/span>\u00a0the\u00a0<\/span>time. I don’t believe him.” Aleta studied her fingernails\u00a0<\/span>for a moment. “Well,\u00a0<\/span>like<\/span>\u00a0I\u00a0said, maybe it’s for the best.\u00a0<\/span>He’s absolutely no use to me now.”<\/span><\/span><\/span><\/p>\n


The practitioner\u00a0<\/span>asked<\/span>\u00a0Aleta to say a\u00a0<\/span>little<\/span>\u00a0more\u00a0<\/span>about\u00a0<\/span>that.<\/span>\u00a0As it happens,\u00a0<\/span>Aleta’s<\/span>\u00a0parents\u00a0<\/span>had<\/span>\u00a0moved\u00a0<\/span>into<\/span>\u00a0her home three months ago, “so they\u00a0<\/span>could<\/span>\u00a0help\u00a0<\/span>out\u00a0<\/span>while<\/span>\u00a0I\u00a0was\u00a0<\/span>recovering,”<\/span>\u00a0she\u00a0<\/span>explained.<\/span>\u00a0“My dad\u00a0<\/span>works\u00a0<\/span>full-time,<\/span>\u00a0as a barber, but he\u00a0<\/span>finds<\/span>\u00a0the\u00a0<\/span>time<\/span>\u00a0to mow\u00a0<\/span>the<\/span>\u00a0grass,\u00a0<\/span>run<\/span>\u00a0errands,\u00a0<\/span>and<\/span>\u00a0do the house\u00a0<\/span>repairs.\u00a0I’m<\/span>\u00a0starting to get a\u00a0little<\/span>\u00a0behind on my bills,\u00a0and<\/span>\u00a0Dad\u00a0<\/span>tries<\/span>\u00a0to\u00a0<\/span>help<\/span>\u00a0out a\u00a0<\/span>little<\/span>\u00a0in\u00a0<\/span>that<\/span>\u00a0department,\u00a0<\/span>too.”\u00a0Aleta<\/span>\u00a0repeatedly stated she wanted to go back to work; however, she also\u00a0said,<\/span>\u00a0“It’s\u00a0kinda<\/span>\u00a0nice<\/span>\u00a0to\u00a0have<\/span>\u00a0Mom\u00a0and<\/span>\u00a0Dad around\u00a0the<\/span>\u00a0house\u00a0right<\/span>\u00a0now.\u00a0Mom\u00a0<\/span>fixes<\/span>\u00a0all<\/span>\u00a0my\u00a0<\/span>favorite<\/span>\u00a0foods\u2014<\/span>[laughing]<\/span>\u00a0I\u00a0know\u00a0<\/span>I’m<\/span>\u00a0gonna\u00a0gain<\/span>\u00a0ten pounds before she\u00a0leaves<\/span>\u00a0to go back home!”<\/span><\/span><\/span><\/p>\n


“You know, my mom\u00a0<\/span>told<\/span>\u00a0me\u00a0<\/span>that<\/span>\u00a0when she was\u00a0<\/span>younger, she had problems\u00a0<\/span>like<\/span>\u00a0me. I guess when she\u00a0<\/span>was a teenager she had problems with\u00a0<\/span>pain<\/span>\u00a0in her back\u00a0<\/span>for a\u00a0really<\/span>\u00a0long time.\u00a0Isn’t<\/span>\u00a0that weird? Mom said when she was around 20 she had trouble with\u00a0really\u00a0<\/span>bad headaches\u00a0<\/span>and<\/span>\u00a0something\u00a0<\/span>with<\/span>\u00a0her breathing\u2014she\u00a0<\/span>doesn’t\u00a0<\/span>like<\/span>to\u00a0<\/span>talk<\/span>\u00a0about it much.” Aleta related\u00a0<\/span>that\u00a0<\/span>she once asked her Aunt\u00a0Dolly<\/span>\u00a0(her<\/span>\u00a0mother’s oldest sister) about it, and she\u00a0had<\/span>\u00a0just<\/span>\u00a0said, “Oh, it’s\u00a0just\u00a0<\/span>something the Walker women get when they are\u00a0<\/span>young’uns.”\u00a0\u00a0<\/span>When asked\u00a0if\u00a0<\/em>she had experienced any previous\u00a0psychological problems such as\u00a0<\/span>hallucinations,<\/span>\u00a0delu\u00ad<\/span>sions, depression, suicidal ideation, or anxiety, Aleta\u00a0laughed, and said, “My lord, no. Aside from\u00a0<\/span>this<\/span>\u00a0neck\u00a0<\/span>and arm pain, my health has been somewhat good.” She described herself having frequent bouts of low back pain, headaches, and “sometimes my jaw hurts real bad, off and on,” she offered, “but overall I don’t have any serious medical problems.”<\/span><\/span><\/span><\/p>\n

During the relating of her past medical history, the\u00a0<\/span>LPC observed that Aleta’s train of thought had been derailed somehow. Sure enough, she inter\u00adrupted herself, “You know, I\u2019ve been thinking about\u00a0<\/span>the question you\u00a0<\/span>just<\/span>\u00a0asked, the question about\u00a0<\/span>psychological problems. Does sibling rivalry count as a psychological problem?” She continued, “When Chad, my brother, and I were growing up, we fought like we were mortal enemies. I mean we hated each\u00a0other. I always felt\u00a0<\/span>like<\/span>\u00a0Chad was Mom’s favorite, so I\u00a0<\/span>grew up feeling like a Secondhand Rose, it was awful.”\u00a0<\/span>The session continued along these lines for another I5 minutes or so. When the time was up and Aleta was walking out the door, she said to the LPC “You know, it’s really great to have all of my mother’s attention right now because I\u2019m in so much pain.”<\/span><\/span><\/span><\/p>\n
