She is 26 weeks
\npregnant, and is a gravida 3, para 0 (2 spontaneous abortions). Courtney is wearing heavy makeup and
\nhas dark circles under her eyes, she informs the nurse that she has left her abusive husband and is
\nstaying at a woman\u2019s shelter. The nurse notes that Courtney does not make eye contact and has lost
\nweight since her last check-up. During the assessment, the nurse notices finger marks on her right arm
\nand bruises on her abdomen. Courtney is crying and expresses concern about the health of her baby
\nand wants to hear the heartbeat.
\nCase Study: Oksana
\nOksana is 39 years old, and 24 weeks pregnant. Oksana has recently immigrated to Canada with her
\nhusband from Nigeria. She has been married for two years and this is her first pregnancy. Her husband
\naccompanies her to all her prenatal visits, and he is very excited about her pregnancy. She has come to
\nher prenatal appointment with complaints of a headache and puffiness in her hands. She states that she
\nis worried that stress in her job will have a negative effect on the baby. Her job requires that she travels
\na lot and eat out with clients. During the assessment, the nurse notes the following: blood pressure
\n142\/90, pulse 80, respiration 16, urine protein +1.
\nCase Study: Emma
\nEmma is 29 years old and is being seen for a 2-week follow-up post vaginal delivery. Emma gave birth to
\na healthy 7-pound daughter. Emma was adopted and does not know her family history; she is living with
\nher partner Sharon. She is complaining of increased vaginal bleeding. She reports that the bleeding
\nbegan on the fourth day after delivery and has increased in severity each subsequent day. Emma has
\ntaken Ibuprofen daily since delivery and states she has a history of iron deficient anemia due to
\nmenorrhagia.
\nCase Study: Melissa
\nMelissa is 21-years-old living with her partner, James on a limited income. Melissa went into
\nspontaneous labor at 38 weeks gestation with rupture of amniotic membranes. During her labor and
\ndelivery, the fetus was monitored internally. After 14 hours of labor she underwent a non-scheduled
\ncesarean delivery. She gave birth to a healthy 6-pound baby boy. She was discharged from hospital day
\n3 postpartum and is currently being seen by the nurse for her postpartum home visit. Melissa is
\ncomplaining of pain in the pelvic area, chills, and loss of appetite. When questioned by the nurse,
\nMelissa mentions that her discharge has a foul odor.<\/p>\n
________________________
\nCase Study: Courtney presents several risk factors that would require health education. Based on the assessment data, three priority learning needs I identified are:
\nIntimate partner violence (IPV) and its effects on maternal and fetal health. IPV increases risks of placental abruption, preterm labor, low birth weight, and maternal depression (1). Education on safety planning and local resources would be important.
\nNutrition during pregnancy. Courtney has lost weight, indicating potential nutritional deficiencies. Proper nutrition supports fetal growth and development (2). Education on a balanced diet, prenatal vitamins, and community food programs could help.
\nStress management. Stress from the abusive relationship and uncertainty of the future negatively impacts Courtney’s health. Chronic stress is associated with preterm birth and low birthweight (3). Teaching relaxation techniques like yoga, meditation, and accessing counseling services may help.
\nThe overarching learning goal could be: By 36 weeks gestation, Courtney will identify three strategies to reduce stress and maintain good nutrition in order to support a healthy pregnancy outcome.
\nSome sources to support the educational content:
\nSaltzman LE, Johnson CH, Colley Gilbert B, Goodwin MM.
